Position Summary:
Our Case Manager identify, engage, and transport individuals inneed to our clinics so they can receive HIV awareness andprevention education and to ensure they are provided STD and HIVtesting, counseling, and referrals. This program provides riskreduction awareness and advocacy and a different approach to lifethrough street outreach, intervention counseling, and HIVprevention education.
Essential Duties and Responsibilities:
The job duties listed below are representative of the job functionsand are not exclusive. Duties responsibilities, and activities maychange, or new ones may be assigned with or without priornotice. Engage in street and community outreach. Recruit participants for HIV/STD/Hep C testing, treatments, andcounseling services. Engages street contacts in risk reduction discussions. Gives out
HIV/HEP C/STD
risk reduction information. Makes home visits and reaches out to persons/patients that presentan adherence problem to treatment. Ensure data collection in a timely, accurate, and consistentmanner. Distributes preventive materials including safe sex kits. Provide Outreach and Recruitment activities including but notlimited to representing the program and the agency at community andcommittee meetings, providing educational workshops and socialmarketing activities. Deliver patient medication as needed Performs other duties and responsibilities as assigned
Phlebotomy Computer Equipment & Software Skill Requirements:
Full knowledge of Windows Office Suite; particularly Microsoft Wordand Excel Basic understanding of computers
