MAJOR FUNCTIONConduct social determinants of health assessments to residents of Polk County. Provide linkage, resources and support navigating services in the community for those identified needs. Provide case management and assessment of needs through field, office or home interviews for eligibility in the Polk Healthcare Plan and referrals for community resources.
ILLUSTRATIVE DUTIES
Provide comprehensive client support, beginning with thorough assessments conducted through referrals and interviews with residents and their families. Evaluations encompass financial, social, environmental, medical, and familial factors to determine program eligibility and appropriate service disposition. Ongoing case management is provided to assist clients in problem-solving and achieving self-sufficiency, which includes crisis de-escalation, advocacy, and collaborative coordination with community agencies like United Way, food pantries, and housing authorities. Navigating services available through the Health and Social Services Division and community resources is a key function, involving referrals, client education on available services, and acting as a liaison with medical personnel and other social service agencies.
Adherence to compliance standards is essential, requiring meticulous documentation in a shared data system, preparation of reports, and interpretation of regulations such as those related to the Polk HealthCare Plan and HIPAA. Collaboration extends to public officials, law enforcement, and legal professionals to address client needs, and participation in meetings to exchange social services information. This position may require local travel, flexible hours, and a willingness to assist in special projects, including providing support during and after natural disasters.
Must be proficient in Microsoft software applications including Word, PowerPoint, and Excel. Ability to perform independently or as a member of multiple team efforts across diverse groups having responsibilities for providing county services. Ability to multi-task and manage multiple cases at one time. Must have the ability to troubleshoot and problem solve using critical thinking. Must exercise reasonable initiative and independent judgment under general supervision. Ability to establish and maintain an effective working relationship with coworkers, management, officials and the public. Must be self-motivated. Ability to communicate effectively, both orally and in writing. Must have excellent organizational skills.
