Job Summary:
This is a professional position responsible for coordinating and facilitating multi-disciplinary team (MDT) meetings/staffings to collaboratively develop and attempt to reach consensus decisions on placement, service needs and other important life decisions of children and youth being served by Florida’s child welfare system. This position will also facilitate other meetings/staffings as needed.
Essential Functions:
Establish and maintain positive working relationship with HFC customers Coordinate Multi-Disciplinary Team (MDT) meetings/staffings to include Department of Children & Families, case management organizations, GALs, parents, youth, caregivers, family members, community service providers and other person involved to assess options and make decisions in the best interest of the child (ren) Receive and review all incoming referrals for MDTs for placement Maintain an organized tracking system to account for multidisciplinary referrals, meetings and confirm participation of required attendees Provide staffing/meeting invite and meeting notifications to all participants timely Provide supporting documentation to MDT participants Ensures that MDT participants are notified of their responsibility to maintain confidentiality of any information shared during the staffing Assist participants in identification the strengths within the family when determining and establishing goals for the child; confirm best interest factors and specific information related to a child 3 years of age or younger are considered during the multidisciplinary staffing pursuant to

provide notification to the courts through Children’s Legal Services and DCF Regional staff within 48 hours after the conclusion of the meetings when a unanimous consensus could not be reached Facilitate consensus building among MDT participants during meetings/staffings Share with all parties the staffing/meeting notes with the court and participant Develop written reports for the courts that outline the overall decisions of the multidisciplinary meetings and the feedback of each individual participant Document the outcome of MDT meetings in the Florida Safe Families Network (FSFN) System Review Florida Statutes, DCF Operational Procedures, Administrative Rule and policies and ensure participants are aware of confidentiality and HIPAA and identify potential problems related to case specific Cross-train co-workers as needed for coverage for the MDT staffing process Participate in ongoing trainings, team and planning meetings Perform special projects as assigned
Qualifications Required:
Bachelor’s degree in social services or related field, three years of child welfare or other social services experience and group or team meeting facilitation skills, excellent written and oral communication skills, strong organization skills and interpersonal and family engagement skills required.
Qualifications Preferred:
One year of child welfare case management experience, current or previous child welfare certification from the Florida Certification Board, formal training on trauma and resiliency and knowledge of Florida’s child welfare system preferred.
