Job Summary:
Responsible for providing support to Case Managers to ensure case management services, communications, and appropriate documentation is consistently processed, while serving as a liaison for patients, family of patients, and hospital staff to ensure patient satisfaction, experience, and quality of services are delivered.
Required Qualifications:
Require a minimum of two (2) years customer service experience in a healthcare or related setting. Require a High School Diploma, GED Equivalent, or Certificate; Associate’s degree in related field preferred.
Preferred Qualifications:
Prefer basic knowledge of computer systems. Prefer previous healthcare experience with discharge planning activities. Prefer demonstrated ability to accurately document in the medical records. Prefer effective oral and written communication skills. Prefer demonstrated ability in establishing and maintaining effective working relationships. Prefer demonstrated initiative and self-direction.
