Case Manager St. Johns County Council on Aging, Inc. 180 Marine Street, Saint Augustine, FL 32084 CASE MANAGER•INDEPENDENT

LIVING SERVICES DEPARTMENT

St Johns County Council on Aging (COA) 20 hrs/wk with room for increased hours as caseload permits•Monday•

Friday Essential Duties and Responsibilities:

Perform in-home client assessment and reassessment, care plan development, care plan review, and services management for clients 60 years of age and older and their caregivers. Maintain case files and compliance with all COA policies/procedures and Department Of Elder Affairs (DOEA) requirements. Respond to clients’ calls, follow-up on inquiries and services, and provide community referrals including Adult Protective Services (APS). Attend trainings, staff meetings and conferences. Perform related duties as assigned by supervisor.

Qualifications and Work Experience Requirements:

Excellent verbal and written communication skills; ability to effectively communicate with clients. Excellent computer skills in MS Office (Word and Excel), and G Suite Tools (Gmail, Drive, Sheets, and Calendar). Ability to learn computer software programs related to client assessments and services. Must be able to work under pressure and meet deadlines while maintaining a positive attitude. Ability to conduct home visits, develop care plan, schedule follow-up services and be knowledgeable of community resources. Bachelor’s degree in social work, psychology, nursing, or related field required or relative experience may be substituted.

Physical Requirements:

Ability to safely and successfully perform the essential job functions consistent with meeting qualitative and/or measurable productivity standards. Ability to maintain regular, punctual attendance consistent with the COA policy. Must be able to lift and carry up to 20 lbs. Must be able to operate machinery and equipment such as but not limited to: computer, copier, fax. Ability to sit for long periods of time. Must be able to listen and speak clearly.
