This would be full-time / 40+ hours per week. LTSS Service Care Manager Compensation The pay for this position is $28 – $30 per hour to start but can transition to a 68K-70K salary. Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates LTSS Service Care Manager Highlights The required availability for this position is Monday – Friday, 8 AM – 5 PM 80% travel required for member visits Valid driver’s license required LTSS Service Care Manager Responsibilities Evaluate member needs, available resources, and develop service care plans Conduct assessments with members, caregivers, and providers for care plan development Monitor service delivery, coordinate referrals, and follow up with members and caregivers Ensure care plans are executed without gaps and identify functional deficiencies Collaborate with discharge planners, physicians, and healthcare teams for proper coordination Assist members with grievances and appeals as needed Maintain accurate documentation within the system within 24 hours of completion LTSS Service Care Manager Requirements Bachelor’s degree in a healthcare-related field (Psychology, Sociology, etc.) 2+ years of Care Management experience Experience managing a high caseload Long-Term Care Medicaid experience Medicaid/Medicare experience Electronic Medical Health Records experience Home Health experience LTSS Service Care Manager Preferences Discharge Planning experience Proficiency in Microsoft Office Bilingual (Spanish preferred)
