Mental Health Case Coordinator Volusia-Flagler County Coalition for the Homeless 605 North Segrave Street, Daytona Beach, FL 32114 Job Description The Case Manager provides intensive case management and supportive services to chronically homeless individuals living with serious mental illness, substance use disorders, and co-occurring conditions. This role emphasizes housing stabilization, connection to treatment, and long-term recovery by using outreach, engagement, and harm reduction strategies that meet clients where they are and support their transition into permanent housing and improved quality of life. Case Managers facilitate integrated client care across key life domains living, learning, working, and socialization by assessing individual needs, evaluating treatment options, coordinating services, and monitoring progress. Working collaboratively with multidisciplinary team members including physicians, mental health professionals, and supportive networks Case Managers help clients enhance overall functioning by identifying their strengths, challenges, abilities, and preferences, and guiding them toward meaningful, person centered goals. Specific Duties Conduct comprehensive assessments to identify client needs, develop goals, and create individualized service and housing stabilization plans. Coordinate and monitor service delivery in alignment with program guidelines, including authorizations and reauthorizations. Facilitate client access to community resources such as housing, healthcare, benefits, vocational training, and life skills support. Engage chronically homeless individuals through outreach and provide trauma-informed, client-centered case management. Link clients to mental health, substance use, and primary care services, supporting informed decision-making and treatment planning. Maintain ongoing care coordination with multidisciplinary teams, including providers, family, shelters, and outreach teams. Support housing retention by collaborating with landlords and housing agencies and providing advocacy, crisis intervention, and skill-building. Attend court hearings and interdisciplinary meetings as required; communicate client progress and treatment updates. Assist clients in developing natural supports and building social networks. Coordinate or provide transportation to facilitate service access. Ensure accurate, timely documentation in compliance with agency, HUD, and funding requirements. Adhere to safety protocols, program policies, and professional standards. Provide administrative and office support as needed. Perform other duties as assigned by program leadership.
Minimum Qualifications:
Experience /
Education:
Bachelor’s Degree from an accredited institution or college with major in Social Work, Counseling, Criminal Justice or any other human service field required. •A minimum of 2 years of full-time or equivalent working experience working with the target population is required. Additional certification in social services field such as Soar is preferable. Knowledge and Skills Skilled in developing and implementing individualized service and housing stabilization plans. Ability to navigate and coordinate complex systems of care, including healthcare, housing, legal, and social services. Excellent crisis management, de-escalation, and problem-solving skills. Strong communication and interpersonal skills for building trust with vulnerable populations and collaborating with multidisciplinary teams. High level of cultural competence and commitment to person-centered, equitable care. Accurate and timely documentation skills. familiarity with HMIS, EHRs, and HUD or grant reporting standards. Capable of balancing fieldwork with administrative tasks, including scheduling, compliance, and data entry. Adaptability, initiative, and resilience in high-demand, community-based environments.
Other Requirements:
A valid driver’s license and reliable transportation are required for the position. Criminal background checks and fingerprinting are required. Ability to work flexible hours to meet client needs. Ability to perform duties in clients’ homes or other community settings as needed.
Compensation and Benefits:
Pay :
40,000-50,000 •This is a grant funded position. Salary for this job is contingent on grant funding availability and/or renewal based on performance.
