Summary We are seeking a compassionate and dedicated Case Manager to join our team at Star Choices, Inc. In this role, you will provide personalized support and guidance to assist individuals with disabilities. Responsibilities Assist in accessing community resources and services. Monitor progress and adjust plans as necessary to ensure optimal outcomes. Advocate for the rights and needs of people supported. Maintain accurate documentation of services provided and client interactions. Oversight of Private Home Care. Provide phone duty relief. Participate in weekly team meetings. Participate in monthly DSP meetings. Provide direct support as needed. Requirements High School diploma or GED required, Bachelor’s degree strongly preferred. At least 1 year of experience working with individuals with intellectual and developmental disabilities is highly desirable. Must also be able to learn relevant software applications, including electronic medical records Dependable Vehicle Valid Georgia Driver’s License and good driving record Current Auto Insurance Acceptable drug screen and background check.
