Full Time Position Summary The HOT (Homeless Outreach Team) Case Manager operates in line with CSAH’s core values of compassion, integrity, and unity . This position provides outreach and case management services to individuals experiencing homelessness, many of whom are also affected by trauma, serious mental illness, substance use disorders, or other complex challenges. The HOT Case Manager engages with clients in a respectful, empathetic, and non-judgmental manner, offering support and resources guided by the Housing First Philosophy, which empowers individuals to pursue self-determined pathways to stability and housing success. This role requires excellent interpersonal communication, cultural competency, flexibility, and the ability to work independently and collaboratively in fast-paced, high-demand situations.

Qualifications Minimum Requirements:

Bachelor’s degree in social work, human services, psychology, or a related field, or equivalent experience providing direct services to vulnerable populations. Demonstrated ability to establish collaborative relationships with community partners, colleagues, and volunteers. Strong crisis intervention skills, knowledge of community resources, and customer service orientation. Ability to work independently and manage multiple tasks simultaneously. Experience working with individuals experiencing homelessness and co-occurring disorders. Ability to model appropriate behavior and mentor colleagues. Conflict resolution skills and creative problem-solving approaches. Job Responsibilities Conduct comprehensive intakes and needs assessments with clients; develop individualized self-sufficiency and housing plans. Provide ongoing case management, including coordination of services and referrals to improve client health, wellness, housing stability, and income. Initiate and support client housing applications tailored to their preferences and functionality, aiming for the least restrictive, stable permanent housing. Provide after-hours outreach and transportation when necessary to clients experiencing homelessness, particularly those with serious mental illness or co-occurring disorders. Connect clients with services such as mental health treatment, Assertive Community Treatment (ACT), Intensive Case Management (ICM), and other essential resources. Promote a culturally responsive and inclusive environment centered on equity and dignity. Respond promptly to client needs and communications (calls, emails, referrals), maintaining positive and professional interactions. Perform data entry and client enrollment using ClientTrack or HMIS; accurately document client progress in accordance with CSAH standards. Maintain complete and up-to-date client charts and records. Participate in quality assurance efforts, including peer chart reviews and 90-day case audits. Submit monthly documentation and statistical reports on client demographics, services provided, and program performance outcomes. Administer and collect client satisfaction surveys and participate in internal program evaluations. Represent the program at internal and external meetings, presenting client concerns and updates as needed. Attend and participate in staff meetings, trainings, workshops, and in-service sessions. Ability to lift up to 50 pounds and stand for extended periods (up to ¾ of the workday). Work in varying weather conditions during street outreach or field-based services. Maintain physical and mental stamina to work in a fast-paced, multi-tasked environment. Travel as needed to support outreach and client services. Work flexible hours, including evenings or weekends, to meet client needs. Perform all other duties as assigned.
