M-F 9-5, On-call Rotation Salary range: $47,000.00- $52,000 based on experience and license

Minimum Qualifications:

Three years of experience in social services, crisis intervention, case management, and social justice, or a combination of education and experience in these fields.

Preferred Qualifications:

Graduate degree in social work or counseling with accompanying license (LMSW, LAPC, LCSW, LPC). Training and experience in trauma-informed services and intimate partner violence. Experience in individual and group counseling with diverse populations and needs.

High-Risk Team Responsibilities:

As a High-Risk Team Community & Outreach Advocate at Project Safe, Inc., your primary focus will be on the following responsibilities related to high-risk cases: Conduct intake assessments to identify survivors of intimate partner violence at high risk for harm or homicide. Perform comprehensive danger assessments to evaluate the level of risk faced by survivors. Develop and implement safety plans tailored to the specific needs of high-risk survivors, coordinating services to ensure their protection. Provide crisis intervention and short term counseling to high-risk survivors to address their immediate needs. Offer intensive case management to high-risk survivors, closely monitoring their progress and safety. Facilitate information and referral services for high-risk survivors to connect them with vital resources. Participate actively in high-risk team meetings, contributing expertise and insights to address complex cases. Collaborate closely with law enforcement, civil and criminal attorneys, healthcare providers, substance abuse counselors, school and childcare providers, DFCS, and other relevant agencies to ensure the safety and well-being of high-risk survivors. Enhance relationships with community partners, fostering a coordinated response to high-risk cases. Document service delivery for high-risk survivors meticulously and provide timely and accurate reports to track their progress.

General Responsibilities:

In addition to your high-risk team duties, you will also be responsible for the following tasks as part of your role: Provide services to survivors and their families in the form of case management, legal advocacy, crisis intervention, emotional support, and psychoeducation utilizing trauma-informed practices. Maintain a caseload of survivors, working towards established goals and documenting services provided and progress towards outlined goals. Stay informed about community resources and make appropriate referrals based on survivors’ needs and eligibility. Collaborate and coordinate with various community agencies, maintaining confidentiality of survivors and fostering positive working relationships within the community. Serve on multidisciplinary teams with partner agencies to assess survivor needs, plan community responses, and provide collaborative care. Facilitate support groups with appropriate activities to foster healing, skill-building, and psychoeducation for survivors. Accompany survivors to legal proceedings in civil, criminal, and juvenile court, assist survivors in filing civil protective orders, and coordinate and collaborate with attorneys and court personnel.
