This position focuses on providing targeted assistance before and after a client secures housing—particularly during the crucial first year of tenancy. In partnership with the Housing Support Coordinator, the Navigator will assess needs and determine when and how additional support should be activated. Role Scope The HSN delivers time-limited, stability-focused services to households experiencing homelessness or at risk of housing instability who are not receiving sustained support. The Navigator plays an essential role in bridging service gaps by offering proactive guidance during both the housing search and tenancy phases. By addressing barriers early and supporting tenants post-placement, the Navigator helps lay the groundwork for lasting housing success. Critical Role Outcomes Serve as the primary point of contact for clients with limited or no case management services. Conduct comprehensive assessments to evaluate housing stability and self-sufficiency. Develop and monitor individualized housing support plans, identifying goals, needs, interventions, and resources. Provide consistent follow-up (weekly, bi-weekly, or monthly) based on client needs. Manage a caseload of referrals from intake to resolution, including thorough documentation. Enter case notes and progress into tracking databases in real time (concurrent documentation). Collaborate with landlords, property managers, and housing navigators to support successful tenancy. Participate in meetings, training, and community events as directed. Remain adaptable and responsive to program pivots, pilot projects, and evolving priorities. Open Doors Job Description Begin working on housing stability at onset of being placed at a property or when additional support is needed due to having no on-going case management services by non-profit agency. Manage each referral assigned to caseload from inception to resolution. Enter all referral progress documentation into Salesforce database. Submit all programmatic and position-related reports within specified timeframes. Attend all assigned team meetings, community meetings, training, and other events as assigned by the Programs Coordinator or Director.

POSITION SPECIFICATIONS

Critical Experiences Background in case management and familiarity with wraparound services (rental, utility, food, mental/physical health, employment). The ability to engage clients in strengths-based planning to overcome housing barriers and sustain tenancy. Respond promptly to referrals and escalations with housing-focused solutions. Comfortable initiating housing stability planning at property placement or re-engagement points. Experienced in working with housing programs such as Section 8, RRH, PSH, VASH, SSVF, LIHTC, and others. Strong collaboration skills with nonprofit agencies, community partners, and housing providers. Bachelor’s degree in communications, Business, Marketing, Social Work, Human Services, or a related field. Public speaking or presentation skills, especially to promote program value. Strategic thinker and self-starter, eager to propose new ideas to increase housing access or funding for vulnerable populations. $50,000
