The Case Manager will travel from their respective home to the client’s home would be compensated at the rate of fifty cents per mile. Each Case Manager is provided with a laptop and will be involved with ongoing training. The Case Manager will have a caseload assigned to them of homebound and Senior Center clients.

Requirements:

Bachelor Degree level of Education.
