at Apple Community Health Services located in Atlanta, GA. Case Manager must be able to cover West Fulton and South Fulton area. The Waiver Case Manager responsibilities include, but are not limited to: Managing collaborating and coordinating member services. Collaborating with client to determine service needs and outcomes. Planning, arranging, coordinating, and evaluating service delivery Conducting periodic in-person member assessments. Monitoring client for any significant changes in health and the care provided to assure services are rendered as ordered in the CCP Acting as client advocate in the human service bureaucracy and brokering Traditional/ Enhanced EDWP Case Management services Brokering services with provider agencies. Coordinating with provider agencies, ensuring comprehensive service delivery and thorough communication Communicating with client’s physician or the agency’s Lead Nurse and Medical Director regarding status changes and health issues Assisting the client with obtaining ancillary services such as transportation and durable medical equipment Identifying, resolving and monitoring issues related to quality of care and timely delivery of services Assuring that clients are free from abuse, neglect of care and exploitation by provider’s agents Initiating actions to address events which may adversely affect the client’s health status Initiating discharge planning Work closely with Intake, Nursing and Billing teams to ensure accurate, compliant billing and premier client service delivery. Attend ACHS interdisciplinary Team periodic meetings. Participate in ACHS’S quality assurance and risk management programs. Qualifications Strong organizational and time-management skills Excellent written and verbal communication skills Ability to work independently and as part of a team 2 or more years of case management, service coordination, and program administration experience Familiarity with Medicaid Home and Community-Based Services Waivers Knowledge of community resources and service providers Experience in the healthcare or social services field is a plus Education Bachelor’s degree in social work, psychology, or related field We offer competitive compensation packages and opportunities for professional growth.
