- Position Purpose
- Mercy Care is seeking a compassionate
- Outreach Case Manager
- to engage individuals experiencing homelessness (PEH) and connect them with essential health and housing services. This role is critical to our mission of providing whole-person care to Atlanta’s most vulnerable populations.
- What You Will Do
- + Conduct community outreach at shelters, encampments, and other locations to engage individuals experiencing homelessness.
+ Assess client needs and develop individualized case plans focused on healthcare, housing, employment, and supportive services. + Provide crisis intervention and assist clients in accessing Mercy Care’s medical, behavioral health, and substance use services. + Collaborate with community partners and Continuums of Care to ensure coordinated, client-centered care. + Support clients with documentation and applications for housing, public benefits, and assistance programs. + Maintain accurate, timely case documentation and participate in regular team meetings and trainings. + Promote harm reduction and trauma-informed care throughout client interactions. + Use company vehicles, including a 12-passenger van, for outreach activities.
- Minimum Qualifications
- + Bachelor’s degree in Social Work, Psychology, Sociology, or related field — OR — at least 3 years of relevant experience.
+ Experience working with PEH, mental health, or substance use populations preferred. + Knowledge of local resources and services for vulnerable populations. + Ability to work independently and as part of a team in a fast-paced environment. + Strong communication, organizational, and interpersonal skills. + Compassionate, non-judgmental, and culturally sensitive approach to working with clients. + Ability to maintain confidentiality and follow ethical guidelines in all professional interactions. + Proficient in Microsoft Office and case management software. + Valid Georgia driver’s license required.
