Job Details About Zaban Paradies Center An independent 501(c)(3) nonprofit organization since 2009, Zaban Paradies Center (ZPC) was founded in 1984 by The Temple as the first and only shelter for couples experiencing homelessness in Atlanta. The mission of the Center was to keep couples together as they moved from homelessness to finding permanent places to live. Currently, ZPC operates a 15room Emergency Shelter for couples and has expanded its programming in recent years to include Rapid ReHousing, Outreach, Day Services, and Financial Assistance programs. These programs have allowed ZPC to dramatically increase its impact in the city and continue to make homelessness rare, brief, and only one time. Having expanded our services and client base, ZPC’s current mission is empowering couples, families, and individuals facing homelessness to achieve selfsufficiency. Zaban Paradies Center is located in Midtown Atlanta at 1605 Peachtree St. NE, Atlanta, GA 30309. This position works 100% inperson from ZPC’s office.

Job:

Case Manager ZPC’s Case Manager for the Residential program is a key staff member who provides vital support services and linkages to community resources to residents of ZPC’s Emergency Shelter in support of ZPC’s goal to assist each couple with identifying a path to permanent housing and empowering the couples to achieve selfsufficiency upon exit from the Emergency Shelter. The Residential Case Manager also provides lighttouch case management and linkages to community resources to Day Services participants. Our shelter is open 7 days per week and provides shelter to 15 couples at a time, and our Day Services program services an average of 1012 participants per week. ZPC is focused on providing solutions to end homelessness in coordination with a number of partners, and the Case Manager will help ZPC achieve its goals. The Case Manager will focus their attention on providing excellent service to residents and program participants, facilitating pathways to permanent housing, and coordinating services with fellow team members and community partners. Responsibilities Case Management Services Provide excellent customer service to ZPC residents, program participants, community members seeking assistance, housing providers, and community partners. Foster an environment of collaboration, professionalism, respect, and accountability. Provide feedback and recommendations to the Center Manager and other relevant staff on client screening processes and prioritization, intake, acceptance, enrollment, service provision, extensions, and exit. Coordinate intake processes and enroll new residents. Comply with established timelines and processes for returning client calls, waitlist management, scheduling, and enrolling new residents. Be mindful of ZPC’s upcoming vacancies to plan for filling open rooms within 7 days of the exit of previous residents. Assist with the development of community partnerships to identify couples and coordinate referral processes. Coordinate with ZPC Assistance Coordinators and other frontline staff to uphold standards for timely, professional response to requests for assistance from calls, online requests, and referrals. Provide referrals for new residents to be tested for TB within 7 days of program entry. Enroll new residents in HMIS after initial 7day probationary period is completed. Coordinate with new residents to ensure that they are connected to a Coordinated Entry System within 7 days of their first night at the Center.
Collect all relevant and required documentation
Assist clients with ordering vital records and documents and identification, and/or connect with partner agencies who provide documentation assistance to individuals experiencing homelessness.
Provide Homeless Verification Letters in support of linkages to community services and housing assistance programs.
Work closely with all Emergency Shelter residents to create individualized stability plans, integrating plans for exit to permanent housing and actionable steps toward goals.
Prioritize connecting households to Coordinated Entry, permanent housing programs, and other services to support households in achieving selfsufficiency.
Ensure Case Notes, Services, and updates to ISPs are entered in a timely, accurate, and thorough manner.
Submit requests to be reviewed by the Center Manager for financial assistance on behalf of clients preparing to move into permanent housing.
Assist residents and program participants with connection to appropriate employment training programs, staffing agencies, life skills training, certifications, and other employment supports.
Assist residents and program participants with benefits applications and/or connections to support for securing benefits.
Identify potential community partners in service of our residents and program participants.
Work with the Lead Resident Advocate and other program staff in support of each Couple residing in ZPC’s Emergency Shelter to prepare for exits to selfsufficiency, including:
Housing Location
Assistance with housing search
Review of affordability and available viable options
Coordination with potential landlords, including apartment homes, singleroom occupancy providers, housing voucher programs, Permanent Supportive Housing providers, and Rapid ReHousing programs
Application assistance
Financial Assistance coordination
Check delivery to landlords and third parties for movein assistance
Coordination of Community Resources
Assist with referrals to Furniture Bank
Assist with identifying key resources in the neighborhood to aid client’s selfsufficiency, including food banks, transportation and other local resources
SelfSufficiency Preparation
Assist with setup of utilities and other requirements of approved housing
Assist with coordination of housing voucher process, when relevant.
Coordinate with Lead Resident Advocate on budgeting, life skills training, and savings, among other preparations for life postshelter.
Enter case notes in HMIS to track client progress, goals, achievements, challenges, and work performed.
Maintain compliance standards, including:
Ensure tracking of clients served through HMIS and other tracking tools
Adhere to internal policies for HMIS
Maintain data quality.
Assist Day Services participants with connection to Coordinated Entry, emergency shelters, housing assistance programs, and community resources as appropriate based on their individual needs.
Other duties as assigned. Work Hours and Location The Residential Case Manager will work inperson at Zaban Paradies Center. Because this position’s duties are carried out 100% inperson at our Emergency Shelter site, there is no planned workfromhome. The Case Manager is expected to have flexibility in their work hours to meet the needs of The Center and its clients, some of whom work during ZPC’s regular business hours. Work hours are subject to change regularly based on program and coverage needs, as well as client availability. Qualifications The ideal candidate will hold a Bachelor’s or Master’s Degree in Social Work or a related social services field, or a minimum of 4 years of relevant experience may substitute for a degree. Candidates should have significant experience in providing traumainformed services to households experiencing homelessness. Candidates should have a background in social work, case management, psychology, or counseling services, or other related fields. Candidates should demonstrate knowledge of local resources and Coordinated Entry systems for people experiencing homelessness. Familiarity with HMIS is preferred. Candidates should demonstrate proficiency in using a computer, online databases, Microsoft Word and Excel, and written communication. Strong organizational and documentation skills are vital to success in this position, as are high emotional intelligence and maturity to work with participants experiencing trauma.
