Job Overview :
We are seeking a dedicated and compassionate Case Manager to join our team. Responsibilities Conduct comprehensive assessments to identify client needs and goals. Develop and implement individualized service plans in collaboration with clients. Provide ongoing support through motivational interviewing techniques to empower clients. Manage crises effectively by employing crisis management strategies. Mentor clients through challenges related to child welfare and addiction counseling. Maintain accurate documentation of client interactions and progress in compliance with agency policies. Collaborate with community organizations and stakeholders to enhance service delivery. Monitor client progress and adjust service plans as necessary to ensure optimal outcomes. Qualifications Bachelor’s degree in Social Work or a related field is preferred; relevant experience may be considered in lieu of a degree. Must be familiar with CPS investigations.
