Case Manager Pelagie Foundation Conyers, GA 30013 Job Summary We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a crucial role in supporting individuals and families by assessing their needs, developing personalized care plans, and coordinating resources to ensure effective service delivery. This position requires strong interpersonal skills and a commitment to improving the lives of those in need. Responsibilities Conduct comprehensive assessments of clients’ needs and circumstances. Develop, implement, and monitor individualized care plans tailored to each client’s goals. Provide crisis intervention and management support to clients facing immediate challenges. Collaborate with community resources, including social services and healthcare providers, to ensure clients receive necessary support. Facilitate access to programs such as Medicare, Section 8 housing, and addiction counseling services. Mentor clients through the process of achieving their personal goals and improving their overall well-being. Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies. Supervise and guide junior staff or interns in case management practices when applicable. Skills Strong crisis management abilities to effectively handle urgent situations. Knowledge of child welfare practices and regulations. Experience in addiction counseling or related fields is highly desirable. Familiarity with Medicare processes and social work principles. Proficient in crisis intervention techniques to support clients in distress. Ability to utilize motivational interviewing strategies to encourage client engagement. Understanding of housing assistance programs such as Section 8. Excellent mentoring skills to guide clients toward achieving their goals. Strong organizational skills with attention to detail for maintaining accurate records.
