We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized service plans, and facilitating access to resources. This position requires strong interpersonal skills and the ability to navigate complex situations effectively. The Case Manager will work collaboratively with clients to promote their well-being and empower them towards achieving their goals. Responsibilities Conduct comprehensive assessments to identify client needs and strengths. Develop, implement, and monitor individualized service plans tailored to each client’s unique circumstances. Provide crisis management and intervention as necessary, ensuring clients receive immediate support. Facilitate access to community resources, including healthcare, housing, education, and employment services. Collaborate with multidisciplinary teams to coordinate care and services for clients. Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies. Engage in motivational interviewing techniques to encourage client participation in their own care. Provide mentoring and support to clients in areas such as childcare, addiction counseling, and behavioral health. Stay informed about relevant legislation, policies, and best practices related to child welfare and workforce development. Skills Strong crisis management skills with the ability to remain calm under pressure. Knowledge of workforce development strategies and resources. Understanding of child welfare principles and practices. Proficient in motivational interviewing techniques to foster client engagement. Experience in childcare settings or related fields is a plus. Ability to perform crisis intervention effectively when needed. Familiarity with addiction counseling approaches is beneficial. Excellent mentoring skills with a focus on empowering clients. Background in behavioral health is preferred. Knowledge of Medicare processes is advantageous.
