Case Manager/Case Coordinator Reports to: Administrator Overview The Case Manager/Case Coordinator oversees the Direct Service Worker(s) and is directly responsible for coordinating all healthcare case aspects related to the staff, treatment, and programs as well as ensuring quality, professional services with the highest level of respect to ensure the dignity of our clients. Specifically, the Case Manager/Case Coordinator is responsible for: Abiding by and enforcing all agency Policies and Procedures and reviewing annually Assisting with reading progress notes. Documenting unworked clients time. Updating clients ‘Home’ binder annually Maintaining all clients’ binders and ‘Home’ binders to ensure all information is updated and correct Purging clients ‘Home & Time Sheet’ binders quarterly Purging documentation in clients’ charts as needed Attending all required trainings and meetings Assuring that all staff is receiving required training (both orientation and annual) as needed to carry out responsibilities Hire according to the staffing needs and staff for all clients Supervising the staffing for Field Supervisors and Direct Support Workers (DSWs) in the field Coordinating with HR on all staff termination and clients’ discharges Detailed weekly reporting, administrative filing and documentation Conducting all supervisory visits which will be unannounced and utilized to evaluate staff Conducting unannounced supervision visits to DSWs 60 days of hire date and annually thereafter Conducting client check ins for wellness and safety Ensuring that all direct care staff are receiving annual evaluations on their work performance Completing intake packets on all new clients, ensure agreements are sent to providers services, signed and a client binder is created Assisting with the upkeep of clients’ tracker chart Answering phones, transfer or take detailed messages when necessary Effectively communicate with clients and staff regarding complaints, concerns and needs with regards to services and document and address accordingly and timely Attending, participating and in some cases hosting weekly staff meetings Leading central office staff meetings and meetings with non-professional staff, when necessary Attending CPOC meetings and assure services are delivered as per CPOC and/or ISP Providing on-call duties as needed Maintaining workplace safety, hygiene and cleanliness at all times Printing Prior Authorization monthly Operating standard office equipment Communicating effectively with Being a role model for emulating the philosophy of Loving Hearts Being a full-time staff member of Loving Hearts Overseeing all aspects of Loving Hearts operations and services Being available in person or by telecommunication at all times to assure smooth and continuous delivery of services Employing and developing qualified staff Assuring that services are delivered according to the consumer’s CPOC and ISP Effectively communicate with owner, administrators, human resources, staff, peers and support partners Assuring that services meet all of the state and federal regulations Assuring that all required licenses or certifications are maintained Ensuring that billing is conducted as required Assuring that all people supported by Loving Hearts have current plans to support their desires and needs Monitoring and documenting services delivered Requesting team meetings when changes are needed Providing on-call duties as needed Reviewing timesheets and progress notes weekly for accuracy Acting as a liaison between Loving Hearts and families Other duties as assigned by the Administrator.
Qualifications:
Bachelor’s or Master’s degree in human services, including but not limited to: Nursing, Physical Therapy, Social Work, Psychology, Gerontology, Rehabilitation Counseling and Health Care Administration. Bachelor’s degree may be substituted for at least 2 years of experience in working with people who have disabilities and/or people who are elderly Must be a resident of Louisiana Knowledge of the AAHSD and OAAS funded services Current Driver’s license Current vehicle insurance Current vehicle inspection Three valid references (Employment is preferred; personal will be considered) Criminal background check clearance Driving history clearance
