Core duties and responsibilities:
Meet regularly with program clients to assist them with reaching goals as outlined in individualized treatment plan developed by the team clinician and client. Work with recently homeless individuals with vouchers to secure permanent housing, benefits, psychiatric and medical services. Assist clients in identifying triggers, natural supports, coping strategies, and strengthening their resilience. Provide supportive counseling. Document services in electronic health record. Participate in scheduled team meetings and consult regularly with team clinician. Assist in locating housing units, completing application process, and maintaining tenancy.
Education required:
Bachelor’s or Master’s in Psychology, Sociology, Rehabilitation, or Criminal Justice . Master’s in counseling or social work is preferred. Supervision for individuals seeking LPC or LCSW licensure is available.
Licensure required:
CSW, LMSW, or PLPC is preferred but not necessary. Benefits include PTO, holiday pay mileage reimbursement, employer match toward retirement plan, healthcare, vision, and dental. Monthly bonus.
