GENERAL JOB DESCRIPTION

The Reentry Case Manager works as part of the Client Services Team to provide one-on-one case management to individuals released from jail and prison. This position is responsible for providing comprehensive support and coordination services to clients in need by assessing client needs, developing individualized care plans, and connecting clients with appropriate resources and services to meet their goals. The Case Manager serves as an advocate for clients, ensuring they receive the necessary support and assistance upon their return home. It is important that this position views each client as a unique individual with unique needs and priorities. This position reports directly to the Director of Client Services and Executive Director.

DUTIES AND RESPONSIBILITIES

Responsibilities include: Provide one-on-one case management to a caseload of at least 40 clients Complete intake assessments to assess which wraparound services the client is in need of Develop a

Mobility Plan:

Understand the client’s individual goals and set measurable milestones that can be achieved to mark progress Regularly monitor clients’ progress towards goals, reassess needs as necessary, and adjust care plans accordingly Engage with clients both in-person and over the phone to understand current needs and short-term goals, maintaining contact even after the client has reached Alumni status Sign clients up for in-house services (Public Benefits, FIT clinic, Economic Empowerment) Assist clients with overcoming barriers that might stand in the way of them meeting their goals (e.g. transportation, money, child care) Provide support with job development and securing stable employment Create resumes, assist with searching for appropriate jobs, assist with job applications / placements, prepare for interviews Provide support with obtaining stable housing Assistance with the housing search, discussions surrounding budgeting, assistance with housing applications, attending property viewings (if applicable) Communicate with client’s Parole & Probation officers Connect clients with services that cannot be provided in-house by referring to contacts at other organizations (Mental Health support, Substance Use Treatment, Legal Aid) Appear in Orleans Parish Criminal District Court to show visual support to both criminal court Judge and client Schedule follow-up appointments with clients to ensure that milestones are being met Collaborate with the Client Services team to streamline work and improve overall client experience and support Maintain accurate and up-to-date documentation of client interactions, assessments, care plans, referrals, and progress notes Utilize data systems such as Charity Tracker, Google Calendar, Gmail, Levitate, and other online organizational systems to track client’s progress Attend Weekly Staff Meeting and report out on current caseload with data Attend Weekly Case Conferencing to communicate needs as a Client Services team and areas for improvement

SCHEDULE

In person, Monday – Friday from 8:30am – 4:30pm. Evenings and/or weekends occasionally as needed

QUALIFICATIONS
Education:

High school diploma or equivalent.

Experience:

At least two years of case management experience and experience working with at-risk and vulnerable populations; an ability to manage a caseload of at least 20 clients. Must have access to reliable transportation. Individuals with a criminal record are encouraged to apply.

COMPENSATION
