Five years of social services experience; OR
Six years of full-time experience in any field plus two years of social services experience; OR
A bachelor’s degree plus two years of social services experience; OR
An advanced degree in a social sciences field plus one year of social services experience.

EXPERIENCE SUBSTITUTION

Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

NECESSARY SPECIAL REQUIREMENT

Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Job Duties and Other Information

Job Duties:

90% Responsible for assisting with implementation of a multi-disciplinary AOT Program, working with the District Court and Judge, community partners and other entities to support community-based treatment for adults with serious mental illness (SMI) who meet criteria for AOT.
Facilitates rounds/staffing with agencies and organizations to provide discharge planning services for potential AOT participants.
Participates in AOT Team meetings, Steering Committee Meetings and Case Management calls.
Coordinates the completion of all required court documents and ensures they are filed in a timely manner.
Gathers information from collateral contact, manage referrals, and collecting and entering demographics and client-level data for a baseline measure, at 3-months, at 6-months of follow-up, and at participant discharge.
Manages collaboration between core partners, identifying and enrolling participants into the AOT Program, and assisting in development and follow up of recovery plans.
Provides intensive case management, referral linkages, and discharge planning and follow-up services to clients with a history of a serious mental illness (SMI) and treatment adherence issues.
Collaborates with AOT Project Director and Program Evaluator on the monitoring of fidelity of evidence-based practices and facilitates/schedules meetings to ensure they are conducted accurately and timely.
As part of an interdisciplinary team, and within the scope of the team, will be responsible for consultation with other team members on case management issues; developing full range of options in the community settings; rehabilitation self-management and wellness strategies; obtaining treatment updates at least quarterly from the treating providers to determine participant progress, hospitals to determine admission/readmission information, criminal justice agencies for documented arrest or involvement with law enforcement, insurance companies, the participant for satisfaction survey, etc.
10% Attends meetings, workshops, trainings and seminars for continuing education purposes and professional enhancement and development.
Performs any additional duties requested which are not listed in job description but relate to services for AOT grant.

Position-Specific Details:

The Assistive Outpatient Treatment (AOT) Case Manager will be supporting the Terrebonne AOT grant. The AOT grant is a program aimed to reduce the frequency and duration of psychiatric hospitalization, homelessness, incarceration, and interactions with the criminal justice system while improving the health and social outcomes of individuals with serious mental illness (SMI) by providing individuals with linkage to community providers and court ordered treatment services. The AOT Case Manager facilitates linkage and referrals to appropriate evidenced-based services for each individual in order to ensure continuity of care. The AOT Case Manager is also responsible for coordinating/facilitating a number of tasks, such as the completion of all court documentation, ensuring the attendance of all required individuals at the hearing, and the implementation of the discharge plan.
This vacancy is being announced as a Classified Position and will be filled as a Job Appointment that may transfer into a Probationary Appointment at a future date.
