Outreach Case Manager Position Available In Berkshire, Massachusetts

ServiceNet is seeking an experienced, forward-thinking Case Manager tojoin our Shelter and Housing Division. In this role, you will providecompassionate support and advocacy to individuals and families workingto rebuild their lives. You’ll help guide our guests on their journeyfrom homelessness to stability, connecting them to the communityresources and services they need to achieve long-term success.

• KeyResponsibilities:

• Conduct intakes and assessments upon guest entryinto the program
• Support shelter and transitional housing participantsby advocating for access to treatment, social services, medical andvocational support, housing resources, and other tools that promoteself-sufficiency
• Meet weekly with guests (individuals or families) toreview service plans, establish goals, coordinate referrals, andevaluate progress
• Collaborate with shelter staff and guests to helpcreate and maintain a positive residential milieu
• Maintain timely,accurate documentation including case management notes, progressreports, and other required records

• Qualifications:

• Bilingualcandidates (Haitian Creole / French / Spanish) are encouraged to apply
• High school diploma or equivalent required, with additional trainingequivalent to two years of college
• At least one year of experienceworking with individuals affected by poverty, mental health challenges,and/or addiction
• Familiarity with local social service agencies andavailable community resources
• Valid driver’s license and anacceptable motor vehicle record
