Gabriel Care Adult Foster Care has been in business since early 2000s and we service the entire state of Massachusetts. Most of our clients are in the Fall River New Bedford area but have clients all over the state (Worcester, Brockton, Lynn, Lawerance, Springfield, Cape Cod and so on). A driver’s license, a car and a cell phone are necessary for the position.

DRIVING IS REQUIRED

You will be given an iPad and/or laptop to use to do your notes, scheduling, case management and assessments on You will be given a caseload with clients that need home visits either monthly or bimonthly. The number of hours you wish to work determines the number of clients on your caseload.

Scheduling is done by you. Summary As an Adult Foster Care Case Manager, you will be responsible for coordinating comprehensive care for patients in a supportive environment. Reporting to the program director, your role will involve utilizing core skills in patient assessment, managed care, and discharge planning to ensure optimal patient service and care. Your premium experience in critical care and geriatrics will enhance the quality of care provided. Additionally, your relevant skills in both inpatient and outpatient settings will be essential in fostering a holistic approach to patient well-being.
