Job Description

Case Manager Program & Location:

Transitional Support Services (TSS),

New Bedford Education & Experience:

Minimum of High School Diploma orequivalent, Associate’s degree or higher & supervised counselingexperience in substance use treatment or closely related fieldpreferred.

Pay Range:

$22•$27 (experience/education dependent)

Status/Shift:

Part Time•Monday•Friday (3 to 7 PM or 4 to 8 PM) CaseManager Requirements Must present as courteous and professional at alltimes Demonstrated flexibility and ability to perform multiple tasksMust have excellent communication skills, including the ability toorganize ideas in logical and clear fashion Knowledge of basic computerskills and software programs Must have the ability to work with othersin cooperative and collaborative manner An understanding of therelationship between substance use and mental health issues and/or selfhelp programs Understand and maintain therapeutic boundaries Reinforcerules in a respectful manner Comply with Federal Regulations, 42 CFRPart 2, Confidentiality of Substance Use Disorder and HIPAA RegulationsTB/Flu and Covid19 Assessment Certification in the HPAO approvedde-escalation program CPR Certification Successful completion of alltrainings upon hire and annually as required Case Manager Duties ℜsponsibilities Maintain safety and security of residents / facilityEnsure timely contact of Program Manager/Authorities in event of acrisis/emergency Understand and follow established referral procedureand schedule intakes Facilitate intakes of new residents includingsetting up new record and completion of intake paperwork Completion ofInitial Individual Service Plan (ISP) with resident Referrals for neededservices to appropriate agencies/providers including but not limited to:

DTA, SSA, DCF, Medical/Mental Health agencies, etc. Maintain accuratecase files, ensures required Authorizations to release/obtaininformation are signed by residents, prepares legible case managementnotes of each session, documents referrals on designated form, updatesISP?s monthly and ensures periodic evaluations are done timelyAccurately record resident attendance at meetings, appointments, groups,etc. Meet with each resident on a bi-weekly basis to review/evaluateprogress in treatment/recovery to determine if current needs are beingappropriately and satisfactorily met; to revise/update the resident?sIndividual Service Plan as indicated, including the establishing ofadditional goals/objective and the steps to achieve thesegoals/objectives Attend monthly case review with Program Manager Attendclinical supervision, required staff meetings and trainings Ensuretimely completion of Admission/Discharge satisfaction surveys byresidents House driving/transportation All other duties as assigned bythe Program Manager Case Manager Qualifications High School Diploma orGED and one-year experience in substance use treatment field preferredValid Driver?s License
