GENERAL SUMMARY OF POSITION

The Young Parent Program (YPP) is a year-round, full-time initiative designed to support recipients of Transitional Aid to Families with Dependent Children (TAFDC) who are young parents lacking a high school diploma or equivalent, or who are seeking to pursue post-secondary education and training. As a Case Manager, you will play a central role in helping participants reduce reliance on public assistance and move toward long-term economic stability. Through individualized support and collaborative planning, you will guide participants in achieving meaningful progress across three key domains: education and career development, life skills and personal growth, and parenting and family well-being. This role requires a compassionate, strengths-based approach to service delivery, as well as strong coordination, advocacy, and communication skills. The Case Manager will partner closely with each participant to assess needs, set goals, connect to resources, and build the skills and confidence necessary for self-sufficiency and success.

ESSENTIAL JOB FUNCTIONS

• Lead the recruitment, enrollment, and retention efforts for participants in the Young Parents Program (YPP). Collaborate with Department of Transitional Assistance (DTA) Engagement staff through co-case management to support participant success. Conduct comprehensive assessments and facilitate individualized goal setting with YPP participants. Manage and reconcile monthly participation data to ensure program compliance and reporting accuracy. Maintain ongoing, clear, and professional communication with the DTA Contract Manager to ensure alignment with program objectives and contractual requirements. Participate in all required meetings, including provider meetings, co-case management sessions, learning labs, and ad hoc meetings, as needed. Plan, oversee, and implement structured day programming activities tailored to the needs of young parents. Support and empower participants in pursuing pathways to economic mobility through education, vocational training, and job readiness initiatives. Engage parents in building supportive networks and developing the skills necessary to access and maintain public benefits, advocate effectively, and provide safe, healthy, and nurturing environments for their children. Promote resilience and long-term well-being by fostering hope, agency, and personal growth, while helping families break cycles of multigenerational poverty and trauma. Guide participants in accessing community resources and developing sustainable life skills.

COMPETENCIES AND QUALIFICATIONS

Bilingual in Spanish preferred, though not required. High school diploma or equivalent required; an associate’s or bachelor’s degree in social work, human services, or a related field is highly desirable. Prior experience in program coordination, social services, or customer service is strongly preferred. Demonstrated ability to manage time effectively and maintain strong organizational skills in a fast-paced environment. Excellent verbal and written communication skills, with the ability to engage and build rapport with individuals from diverse backgrounds. Compassionate and empathetic approach, with a strong sensitivity to the unique needs of families and young parents. Proven ability to work independently, exercise sound judgment, and manage responsibilities with minimal supervision. Familiarity with scheduling software or case management tools is an asset. Maintains a high level of professionalism, integrity, and discretion in handling confidential information.

PREPARATION, SKILLS & TRAININGS REQUIRED

Required trainings may include CPR/First Aid, Trauma Informed Care/Collaborative & Proactive Solutions (CPS) and/or similar Behavior Management Training, Protective Intervention Training, Medication Administration, Orientation training; Policy Review, HIPAA, OSHA, Human Rights, Safe Driving, Emergency Incident Management, Work Place Violence, Anti-Harassment, Ethics and Boundaries, and any other related trainings offered by Centerboard, Inc. Attendance to Centerboard’s multi-cultural awareness orientation is required participation in an affinity or ongoing multi-cultural awareness training is mandatory. All employees are required to attend refresher training to maintain certification compliance and to exemplify competency of skills for each of the trainings. Required skills include the ability to prioritize tasks, communicate effectively/professionally, flexibility in a changing environment, and strong organizational skills.
