Case Manager Description We’re seeking a Case Manager to join MHA’sRecovery and Housing Division, within our Homeless Outreach/HHG program.
The Case Manager supports adults experiencing homelessness or at risk ofbecoming homeless by assisting them in obtaining and maintainingindependent subsidized housing. This role promotes advocacy,skill-building, and community engagement for program participants.
Responsibilities are divided between office work and fieldwork. Whilethe standard schedule is Monday through Friday, occasional flexibilityfor weekend hours is required. Essential Functions Support for
ProgramParticipants:
Gathering necessary documentation for housingapplications. Assisting in identifying suitable housing through onlineapplications and landlord outreach. Maintaining housing through advocacyand mediation. Providing referrals and connections to mental healthand/or substance use providers. Teaching and role modeling independentliving skills. Encouraging active community participation throughvolunteer activities. Facilitating access to primary care and wellnessservices. Supporting participants in achieving employment andeducational goals. Assisting with obtaining benefits such as socialsecurity, SNAP, SSI, and others. Helping participants achievesustainable income through regular employment.
AdditionalResponsibilities:
Completing documentation requirements, includingassessments, progress notes, and data entry. Transporting programparticipants as needed. Conducting thorough housing inspections toensure safety and suitability. Requirements A valid driver’s license,sufficient automobile insurance, an acceptable driving record, and asafe, reliable vehicle during working hours. The ability to workindependently, manage time effectively, prioritize tasks, takeinitiative, and collaborate as part of a team. Strong oral and writtencommunication skills, as well as computer and organizationalproficiency. Adaptability to the changing needs of program participants.
Patience, compassion, and strong interpersonal skills. The ability toobtain and maintain certifications in medication administration (MAP),CPR, and first aid (training provided). A minimum age of 18 years old.
Education and Experience A high school diploma or equivalent (GED).Experience working with individuals with disabilities. (Preferred)Experience assisting individuals with housing-related needs. (Preferred)Bilingual abilities. Compassion and empathy, with a strong desire toassist others. Physical Demands Frequent talking, hearing, sitting,typing, and writing. Occasional kneeling, climbing stairs, or liftingobjects up to 40 pounds. Visual perception for inspection andcoordination tasks.
