Job Description
Full-time, 35 hours per week Hybrid up to 3 days per week once trainingis complete. Bilingual Geriatric Support Services coordinatorResponsible for functioning as a case manager within the Primary CareTeam (PCT) for members participating in a Senior Care Option (SCO) plan.
Make recommendations, coordinate, implement, and monitor contracted andcommunity services for the purpose of improving or maintaining themember’s overall health and functional status. Conduct periodicreassessments to determine appropriateness of the service plan andensure that members have access to public benefits to support theirwellness, independence, with the goal of preventing prematureinstitutionalization. Travel reimbursement is $0.62/mile
MinimumQualifications:
BA or BS in Social Work, Human Services or relatedfield, experience in case management with geriatric population. LSW aplus. Must have valid driver’s license and/or reliable transportation.
Spanish/English fluency required. Must be able to climb multiple flightsof stairs.
