If you’re passionate about human services, thrive in a collaborative environment, and want to be part of something meaningful—this is the role for you! ????What You’ll Do As a Case Manager, you’ll be the vital link between youth, families, and the network of support that surrounds them.
You’ll:
???? Build strong, trust-based relationships with clients and their support teams—schools, therapists, foster homes, and more. ???? Set meaningful goals and develop personalized interventions that help youth thrive. ???? Coordinate care plans, monitor progress, and ensure every step aligns with best practices and compliance standards. ???? Keep accurate, timely records and reports that drive informed decision-making. ???? Participate in on-call services and represent the agency in community initiatives and task forces. ???? Stay ahead of the curve with trends in Human Services and Case Management. ????️What You Bring A heart for service and a mind for strategy. Bachelor’s degree in Social Work, Psychology, or a related field. Experience working with children from challenging backgrounds. Training in trauma-informed care (a big plus!). Strong communication, organization, and problem-solving skills. Valid driver’s license and personal auto insurance. ????Perks & Benefits ???? Tuition reimbursement to keep you growing. ????
