Position Summary:
The individual in this position works under the direction of the RNand/or Social Worker to complete referrals for post-acute servicesfor patients, and assists with tasks that do not require a clinicallicense or degree. The individual’s responsibilities include thefollowing activities: a) arrange post-discharge services; b) createand follow up on electronic referrals using the Tenet CaseManagement system; c) review patient choice letters withpatients/families for required signatures; d) provide follow upImportant Message to Medicare patients prior to discharge; e)communicate with patients, families and other members of the careteam; f) complete tasks assigned by Case Manager and Social Workstaff; g) make copies, send faxes and complete phone calls; h)complete process reviews or audits as requested; and i) otherduties as assigned. Responsibilities Makes referrals for post-acuteservices under the direction of the RN Case Manager or Social Work(SW) staff, utilizing the electronic Tenet Case Management system.
Provides patients and families with choices of post-acute providersper Tenet policy. Responds to post-acute providers timely, andcompletes referrals per Tenet policy. Documents and communicatesall elements of the post-acute referral to the RN Case Manager orSW, and the healthcare team, patient/family and post-acuteproviders. Completes tasks as assigned by RN or LVN Case Managerand/or SW staff. Makes copies, send faxes, and complete phone callsto arrange post-acute services and to ensure that appropriatehospital information is communicated to post-acute providers.
Documents all referrals and tasks in the Tenet Case Managementsystem per Tenet policy. Provides patients and healthcare teaminformation regarding resources and benefits available to thepatient along with the economic impact of care options. Compliance-Adheres to federal, state, and local regulations and accreditationrequirements impacting case management scope of services. Adheresto department structure and staffing, policies, and procedures tocomply with the CMS Conditions of Participation and Tenet policies.

2 years of experience in clericalor healthcare field.

Acute hospital experience.
