Position Overview:
We are seeking a compassionate and highly motivated Master’s Level Case Clinician to join our dynamic team. The ideal candidate will be responsible for providing client-centered case management services to individuals especially migrants facing various life challenges. This position does not require a professional license but requires a Master’s degree in a relevant field such as Social Work, Psychology, Counseling, or related disciplines. The Case Manager will work closely with clients to assess their needs( help client find jobs, create resume etc) , create personalized care plans, and connect them to community resources, ensuring they receive the necessary services to improve their overall well-being.
Key Responsibilities:
Conduct thorough assessments to identify client needs, strengths, and challenges. Develop and implement individualized care plans in collaboration with clients. Assist clients in accessing social services, healthcare, housing, and other necessary resources. Monitor client progress and make adjustments to care plans as necessary. Provide ongoing support, guidance, and advocacy to ensure clients’ needs are being met. Maintain accurate and up-to-date case records and documentation. Coordinate services between multiple agencies and providers to enhance client outcomes. Provide crisis intervention and support in emergencies as needed. Participate in multidisciplinary team meetings to discuss client progress and treatment plans. Educate clients and their families about available services and self-advocacy. Ensure compliance with relevant policies, regulations, and confidentiality standards.
Qualifications:
Master’s degree in Social Work, Psychology, Counseling, Sociology, or a related field. No professional licensure required, though experience or education in a related field is preferred. Strong interpersonal and communication skills, with the ability to work effectively with diverse populations, especially migrants . Ability to assess client needs and help client meet the need and develop appropriate care plans. Knowledge of local community resources and social service systems. Excellent organizational and time-management skills. Ability to work independently and as part of a team. Strong problem-solving and crisis management abilities.
