Summary:

The Case Manager coordinates, oversees and evaluates the provision of social services to participants/clients/members of the Adult Foster Care and Group Adult Foster Care programs. The Case Manager also plans and implements social service interventions.

Responsibilities:

Conducts psychoanalysis assessments Coordinate social services with program director Assists with admissions and orientations Conducts AFC Setting in conjunction with MassHealth standards for intake and assessment process for new clients/members Conducts visits weekly, monthly and bi-monthly per MassHealth regulations: Provides social services in accordance with the plan of care, and in coordination with the multidisciplinary team. Makes the initial and or comprehensive physco-social evaluation visit, determines primary focus of care, assist in the develop of the plan of care within MassHealth regulations, and submits accurate, complete, and timely documentation, per policy. Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 30 or 60 days) the patient’s nursing needs. Instructs and supervises the patient’s Caregiver in the care of the patient and maintenance of a healthy environment for the patient. Actively participates in MDT meetings or case conference. Communicates changes in visit assignments, dates of scheduled visits, and schedule changes to scheduler and Administrative team to ensure patient needs are met. Performs regular and supervisory visits according to the plan of care and submits complete visit notes within 24-48 hours of completion visit. Directly and/or indirectly supervises care provided by the home health aides and Caregivers, provides instruction as appropriate, and assigns tasks according to MassHealth regulations. Assists in the orientation of new agency personnel as assigned. Completes assignments per DON request, as well as any MassHealth training per state regulations. Participates in the Continuous Quality Improvement (CQI) to ensure positive patient outcomes.
