Seek out and pay attention to the preferences and interests of participants. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks. Essential Duties and Responsibilities (other duties may be assigned) Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills. Provide direct care for ADLs and group activities. Lift and transfer individuals as appropriate to their needs. Coordinate services with Interdisciplinary Team. Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams. Develop and implement Day Habilitation Service Plans (DHSP), including: Maintain progress notes and document progress on objectives Maintain assigned participant records so that all information is current and filed appropriately Write bi-annual and annual summaries Complete all required documentation in a timely and accurate manner. Participate in scheduled supervision sessions with Assistant Director. Participate in program-wide staff meetings and case review meetings as scheduled. Participate in team meetings regarding general program development, including curriculum development and schedules. Ability to demonstrate good computer skills as well ability to learn to utilize the agency’s computer programs for documentation of necessary data. Attend in-service trainings as required. Ability to work in a team environment. Ability to multitask and prioritize tasks. Strong written and verbal communication skills along with excellent interpersonal skills. Comply with all applicable State and Federal regulations, agency policies and procedures, and personnel practices. Protect the health and safety of all participants and staff. Performs other duties as requested by supervisor(s).

Qualifications:

Education and Experience:
BA/BS in Human Services /related field or associate degree plus 3 years related experience with persons with ID/DD or Human Services or HS Diploma/GED plus 5 years of related experience with persons with ID/DD or human services.
Language Skills:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak and present effectively before individuals or employees of an organization.
Mathematical Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability:
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Vehicle and Travel:
Requires a legally insured and inspected automobile for use to be presented at work and available for use during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.
Certificates, Licenses and Registrations:
Valid and current driver’s license from the state of residency.
