Independent Community Supports (ISCH) Specialist – Worcester & Surrounding Areas Prime Home Health and Companion Care is looking for a compassionate, organized, and community-driven ISCH Specialist to join our growing team. This role focuses on supporting individuals with disabilities who are living independently in the community, helping them maintain stability, safety, and quality of life at home. As an ISCH Specialist, you’ll work directly with clients to assist with everyday needs and long-term goals that promote self-sufficiency and well-being. Tasks may include coordinating transportation, supporting with shopping or errands, attending medical appointments, helping clients connect with community resources, organizing paperwork for public benefits like SNAP or fuel assistance, and budgeting household expenses. The right candidate is someone who enjoys problem-solving, has strong communication and time management skills, and is comfortable working independently in the field. You’ll also be expected to document your visits, stay in contact with your supervisor and case managers, and attend team meetings as needed. Experience working in community support or human services is preferred, and a bachelor’s degree or licensure in a related field is a plus. This position offers part-time hours to start with the potential to grow into a full-time role. Scheduling is flexible, and the position includes both fieldwork and remote administrative tasks. Requirements:
Associate’s degree, bachelor’s degree, or licensure in a related field preferred, but equivalent community experience will be considered Preferred experience in transitional assistance and housing support. Experience in community services, case management, or related fields. Passion for working with frail and/or elderly individuals in a home setting. Exceptional communication skills for interacting with clients, families, and other professionals. Strong documentation and organizational skills. Proficiency in assisting with applications for state benefits such as SNAP. Ability to assist with budgeting, shopping, and finding community resources such as medical providers and day programs Competence in attending meetings and coordinating services with clients, families, case managers, etc. Comfortable with arranging transportation, viewing housing, and scheduling appointments. Flexibility to work part-remote and adapt to a fluctuating schedule. Availability to work part-time initially, with the potential for full-time hours soon. Willingness to travel within the southeast Massachusetts area Reliable transportation for traveling to client locations. A commitment to building safe, comfortable, and supportive household environments for clients transitioning to home care. Bilingual in English and Spanish are highly encouraged to apply.
