to adults who are facing challenges that affect their quality of life,
such as mental health issues, substance abuse, disability, homelessness,
or other social service needs. The primary role of the case manager is
to assess client needs, develop individualized service plans, connect
clients to necessary resources, and monitor their progress. Qualifications | Education | Certification Minimum of MHRT/C provisional certification required Associates Degree in related field, full MHRT/C, or two years
experience preferred Excellent written, verbal, interpersonal, listening, and
communication skills Ability to handle multiple tasks and prioritize continually changing
tasks Be dependable and reliable Basic mathematics skills Provide high-quality, compassionate care Problem-solving, organizational, and time management skills are
essential Independence in decision-making and collaboration with supervisors
expected Must be able to work under conditions involving emotionally
stressful situations Must be able to meet strict guidelines and timeframes for
documentation Participation in weekly supervision is required This position may require travel within the community to meet with
clients and attend meetings, and as a result a valid driver’s license,
automobile liability insurance, and safe and legal transportation is
required.
