Case Managers support clients by advocating for them in the community, helping them to identify needs and goals and connecting them with appropriate community and Homeworthy resources. Case Managers at Homeworthy are part of helping to achieve the agency’s mission on a daily basis. They help to identify and build on strengths, always treat clients with dignity and respect, and ultimately become trusted partners who help clients to reach sustainable independence.

Education:
1. Bachelor’s Degree from an accredited four (4) year institution of higher learning, with at least one (1) year of human services or related experience; OR, 2. Master’s Degree in Social Work, Education, Psychology, Counseling, Nursing or other human services field.

Excellent organizational, critical thinking, and problem-solving skills, with attention to detail 4. Strong interpersonal and listening skills, able to communicate effectively in diverse settings and interact respectfully with a wide range of ages, backgrounds, and personalities 5. Ability to manage multiple appointments and manage time, effectively; 6. Ability to multitask and remain focused in a busy work environment; 7. Solid research skills; 8. Thorough knowledge of community resources; 9. Strong communication skills, both written and verbal; 10. Knowledge of basic computer skills; proficiency with Microsoft Office Suite and Google Suite, preferred.
Licenses and Experience:
1. At least one (1) year of case management, social work or related human services experience 2. Experience working in a team setting 3. Valid driver’s license and automobile insurance 4. Current American Red Cross CPR/First Aid Certification

Tasks:
- Relate to clients and team in a professional, respectful and caring manner;
- Maintain confidentiality of all information related to clients and/or the organization, except as it pertains to mandated reporting laws;
- Adhere to statewide mandated reporting laws, as they relate to the abuse of children or dependent adults and/or to clients who intend to cause severe, bodily harm to themselves or others;
- Perform client intake assessments, as needed;
- Respond to client communications in a timely manner; be proactive about reaching out to and following up with clients;
- Maintain a working knowledge of the needs of vulnerable populations, e.g. chemically dependent, mentally ill, victims of abuse, etc.;
- Maintain a thorough knowledge of local resources; develop and maintain positive relationships with community providers;
- Refer/connect clients to appropriate services, including (but not limited to) housing, mental health, substance abuse, child welfare, employment and education;
- Help clients secure reliable transportation;
- Work collaboratively with clients to establish and prioritize client needs and goals; create an integrative plan of care, which may be revised with the client, as needed;
- Get to know each client’s psychosocial history, including client strengths, challenges, fears/phobias, triggers, interests and coping mechanisms;
- Regularly meet and/or follow up with all clients on caseload; if a client is habitually non-communicative, then seek supervision;
- Connect clients with local crisis services, as needed; work actively with community providers to support clients who are in crisis;
- Use reflective listening skills to establish trust, validate client feelings and check CM observations (for example, to check observations about a client’s non-verbal communication or level of participation);
- Meet regularly with Case Management Supervisor and Clinical Supervisor to review cases, problem solve, ask questions and receive feedback;
- Always stay within case management scope of practice; when unsure about scope of practice, seek supervision;
- Maintain client and program records in accordance with organization standards, applicable state laws, MaineCare regulations, and Maine State Housing Authority (MSHA) requirements; complete all TCM paperwork and reassessments by specified deadlines;
- Complete and sign progress notes, weekly;

- Participate in required agency activities and initiatives, such as on or off site employee trainings and staff meetings;
- Be knowledgeable about the programs and services that Homeworthy offers, beyond case management (i.e. shelter, mental health, transportation, laundry, etc.) and about how to properly access those services; be proactive about seeking assistance, when unsure about the rules and guidelines of a Homeworthy program or service.
- Perform other duties, as needed, which are necessary to ensure departmental success.
Physical Environment:
Duties involve working both in and out of an office setting; duties may involve meeting with clients at their residence(s), in public spaces, and in and around program spaces, including Homeworthy facilities. Travel may be required throughout the Midcoast region.
Physical Effort:
This is a position that requires extensive communication with clients, by telephone and in person. The position may involve standing, sitting, bending, reaching, typing, filing, lifting up to 30 pounds, ascending and descending stairs, and driving.
