We are looking for a Case manager to enhance the quality of patientmanagement, maximize satisfaction and promote cost effectiveness. Thesuccessful candidate will be accountable for the full patient carecycle. Case manager duties include assessing, planning, implementing,monitoring and evaluating actions required to meet the clients healthand human services needed. A certified case manager is a plus.
- Responsibilities
- Provides personnel case management services for adults with developmental disabilities, to include developing an individual support plan (ISP) and assisting the individual with implementing that plan
- Develop and/or assist in the development of the Person-Centered Plan (PCP
- Coordinate all necessary services for the individual as outlined in the PCP to include but not limited to medical, dental, mental health, housing, finances, personal, social, and emotional supports, community integration, and all other services
- Assures compliance with applicable federal and state regulations and laws
- Maintains philosophy, policies and procedures outlined in the Uplift, Inc. policy manual
- Assures adequate records are maintained to include the Initial Assessment, PCP, ISP, contact notes, accurate use of Therap, as well as all other forms of written communication and documentation.
