A successful candidate will be a dedicated and enthusiastic individual who believes the mission of the agency is vital to the success of individuals pursuing long-term recovery. The ideal candidate will be responsible for assessing, planning, and coordinating care to meet the needs of each client. The candidate will work closely with a multidisciplinary team to support clients and help them become self-sufficient following their treatment.

Responsibilities:

Support, teach, and encourage the client to achieve independence. Develop a plan for the client that reflects his/her goals and is developed in partnership with the client and treatment team; and periodic reviews of the plan of care. Complete comprehensive assessments of client’s needs and desires, while also identifying and communicating client’s medical needs and arranging for appropriate medical care. Work collaboratively with mental health services staff, treatment team members, and other service providers regarding care plans. Preferable experience with navigating and completing FMLA and STD paperwork. Assist clients in developing a comprehensive aftercare plan; including identifying outpatient providers and assisting with obtaining appointments. Assist clients in developing resumes and seeking employment. Assist client in communication with attorneys and court systems to navigate pending and ongoing legal issues. Provide clients with necessary letters and documents as needed for psychosocial issues. Participate in multidisciplinary team meetings and share about client needs/progress. Coordinate with providers on client care. Facilitate psychoeducational group sessions. Document client progress notes following appointment. Document group session notes.

Qualifications:

Bachelor’s degree in Human Services, Psychology, Social Work, or a related field. Experience in case management or care coordination. Strong communication and active listening skills. Empathy, patience, and the ability to establish rapport with clients. Knowledge of substance use recovery. Understanding of co-occurring mental health disorders. Commitment to ongoing professional development and training.

Preferred Qualifications:

Experience working in a diverse and multicultural setting. Experience with electronic health record (EHR) systems. Familiarity with KIPU preferred.
