Position:

Case‑Manager (Part‑Time to Full‑Time, Daytime Weekdays) We’re seeking a superstar : someone knowledgeable about community resources, deeply empathetic, and professional. You meet people where they are—no judgment. Ideal candidates will have strong life experience, organizational skills, and the ability to transport clients safely in their own vehicle. Responsibilities Provide compassionate, person‑centered case management support for clients with traumatic brain injury (TBI). Help clients access community benefits SSDI, SSi, food stamps, Medicaid, housing and more. Use your vehicle to safely transport clients to appointments, community meetings, and essential services. Maintain clear, organized documentation: client records, progress notes, benefit applications, and related paperwork. Build individualized service plans and track progress—acting as both advocate and navigator. Requirements Experience in social work, case management, community support, or a related field—especially with TBI or disability services. Bachelor’s degree preferred, but we’ll strongly consider relevant life and work experience. Valid driver’s license, clean driving and criminal record, current liability insurance on your vehicle. Excellent written and verbal communication, organizational skills, and paperwork proficiency. Flexible availability for daytime weekday hours (max. 40 hours/week).
