POSITION OVERVIEW
The Home Care Case Manager is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/caregiver introductions and quality assurance visits with existing clients. They determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction, and retention. This position works closely with clients and staff to offer support, feedback, coaching, and training. This position will support the Overland Park, KS and Kansas City, MO Offices.
RESPONSIBILITIES
This position serves as a first touch point for new clients, and as an account manager for existing clients. In addition, the position plays a key role in identifying client needs and matching appropriate services. New Client Intake Assessments
- Conduct prospective new client intake calls, determine client needs, and schedule care assessments. Create new client profile and care plans through the entry of accurate client data into FirstLight Home Care software systems Case Management
- Perform ongoing in-home evaluation, caregiver introductions, coordination, and supervision of client services Supervise and coach caregivers and assist clients in obtaining needed resources to remain safely at home.
Accurately identify and report changes in client condition to agency employees involved in the client’s care plan Participate in the quality assurance reviews and evaluations of the agency’s services Express proficiency in all FirstLight Home Care services, policies, benefits, and market differentiators. Work toward continuous quality improvement and provide unparalleled customer service for all FirstLight Home Care clients.
EXPERIENCE
Experience in home care, home health, hospice, or senior-related industry highly preferred
- an equivalent combination of education and work experience may be considered.
Bachelor’s Degree is Social Work, Nursing or Health Administration highly preferred Working knowledge of HCBS services including home care, home health, Medicare, Medicaid, VA, etc… Familiarity and comfortable training staff for ADL’s, IADL’s and other common service needs for seniors.
