Counselor/Case Manager Position Available In Hinds, Mississippi

Counselor/Case Manager West Jackson Community Development Corporation 1635 Raymond Road, Jackson, MS 39204

Key Responsibilities:

1. Case Management & Counseling Conduct intakes, needs assessments, and create Individual Development Plans (IDPs) Provide one-on-one counseling sessions to address social-emotional, behavioral, or life challenges Refer participants to outside services as needed (mental health, housing, substance abuse, etc.) Track participant progress and follow up consistently 2. Supportive Services Coordination Connect students to transportation, childcare, housing, healthcare, and other supports Collaborate with community organizations to build referral pipelines 3. Attendance & Retention Support Monitor attendance and participation across education, vocational training, and leadership components Conduct outreach and home visits when necessary Work with staff to develop re-engagement plans for disconnected students 4. Record Keeping & Documentation Maintain confidential case notes, files, and service logs in compliance with grant or funder requirements Input data into internal and external tracking systems (e.g., Efforts to Outcomes – ETO) 5. Program Support Participate in morning circles, team meetings, leadership development activities, and field trips Serve as an advocate for students during disciplinary or support plan meetings
