Our Brands Responsibilities:
The Foster Care Case Manager is responsible for overseeing theservice planning, counseling, supervision, and case management ofchildren and families within the foster care system. This role iscritical in developing individualized treatment plans, coordinatingcomprehensive services, and ensuring that foster children receivethe medical, social, educational, and other services they need.
Develop and implement individualized treatment plans for childrenin foster care. Prepare and maintain detailed case records andcomplete weekly activities/status reports. Coordinate and linkchildren to appropriate services to minimize care fragmentation andensure continuous access to necessary services. Facilitaterelationships and interactions between birth parents, fosterfamilies, and children to support family success and child welfare.
Monitor and continually evaluate each child’s care plan, adjustingstrategies as necessary to meet evolving needs. Assist in therecruitment, assessment, and training of foster parents. Engage incollaborative relationships with foster parents to help themprovide a safe, nurturing environment for foster children.
Coordinate services and service planning with other agenciesinvolved with the child by arranging medical, remedial, and dentalservices as needed. Collaborate closely with family assessment andplanning teams to prepare all case plans and coordinate services.
Ensure all documentation is completed and submitted withinestablished timeframes in accordance with company policy. Maintaincompliance with Health Connect America (HCA), State, and Council onAccreditation (COA) health and safety requirements. Resolvestakeholder issues promptly and proactively to avoid complications.
Maintain high-quality work standards, focusing on continuousimprovement and quality. Communicate effectively with allstakeholders, keeping them informed and updated. Collaborateeffectively within the team, accepting feedback positively andcontributing to team efficiency.
Qualifications:
Bachelor’s degreein social work or a related field such as sociology, psychology,special education, or counseling. At least two (2) years ofexperience in providing casework services to children and families.
