Job Summary:
Position Summary – CAP CaseManagerHealthkeeperz is seeking a compassionate and detail-orientedCAP Case Manager to join our team, providing essential casemanagement services for the Community Alternatives Program (CAP)for Disabled Adults and Children. The CAP Case Manager plays avital role in ensuring that participants receive the coordinatedmedical, social, and financial services they need to live safelyand independently in their communities.
As a CAP Case Manager, youwill conduct comprehensive assessments, evaluate eligibility forCAP services, and develop individualized care plans tailored toeach patient’s needs. You will collaborate with patients, families,and provider agencies to coordinate care and make informedreferrals to appropriate community resources. This role includesongoing monitoring and documentation of services, regular homevisits and telephone check-ins, and evaluation of careeffectiveness.
The ideal candidate demonstrates strong clinicaljudgment, emotional intelligence, and an ability to build trustwith patients and care providers alike. You will be responsible formaintaining complete and accurate documentation in accordance withagency and state CAP guidelines, including the use of the E-CAPsystem, reviewing In-Home Aide documentation, and supportingcompliant billing practices.

Identify social service needs and connect patients toappropriate community resources. Evaluate eligibility, needs, and support systems.
Planning Develop and update individualized care plans that promotepatient health and safety. Educate patients and families on care plans and availableservice options. Initiate referrals and provide counseling to support family andcaregiver well-being.
Implementation and Follow-Up Coordinate care with interdisciplinary team members andexternal providers. Perform regular follow-ups through home visits and phonecheck-ins. Monitor and document care effectiveness and adjust plans asneeded. Provide monthly case management and attend interagencyconferences as appropriate.
Documentation Maintain accurate, timely records per agency policy and CAPguidelines. Document patient status, changes in condition, and careoutcomes. Complete discharge summaries and ensure all documentation iscurrent in E-CAP. Review aide documentation, re-certifications, and billing codesfor compliance.
Professional Development Complete continuing education and state-mandated training. Stay current with E-CAP system updates and CAP programrequirements. Collaborate with the CAP Director to establish and reviewannual professional goals. Fulfill mandatory organizational training.

Working knowledge of basic social work principles, techniques,and practices and their application to specific casework, groupwork and community problems Knowledge of governmental and private organizations andresources in the community Strong organizational, communication, listening, and assessmentskills Ability to travel as needed to other office locations
Education/Training:
1. Bachelor’s degree in social work from an accredited schoolof social work and one (1) year of directly related communityexperience (preferably case management) in the health or medicalfield directly related to home care, long-term care, or personalcare and the completion of an NC Medicaid certified trainingprogram within 90 calendar days of employment; OR 2. Bachelor’s degree in a human services or equivalent fieldfrom an accredited college or university with two or more years ofcommunity experience (preferably case management) in the health ormedical field directly related to home care, long-term care, orpersonal care and the completion of an NC Medicaid certifiedtraining program within 90 calendar days; OR 3. Bachelor’s degree in a non-human services field from anaccredited college or university with two or more years ofcommunity experience (preferably case management) in the health ormedical field directly related to home care, long-term care, orpersonal care and the completion of an NC Medicaid certifiedtraining program within 90 calendar days; OR 4. Nurse who holds a current North Carolina license with two(2) year or four (4) year degrees and one (1) year case managementin home care, long-term care, personal care, or related workexperience and the completion of an NC Medicaid-certified trainingprogram within 90 calendar days; OR 5. An individual with a bachelor’s degree or who holds anursing license as described above, without the number of years ofexperience, may be designated as an apprentice or a trainee andshall be hired to act in the role of case manager. The supervisorof the case manager shall provide direct supervision and approveall waiver workflow documentation and tasks. •Hybrid Work & Travel Expectations•The CAP Case Manager positionfollows a hybrid work model, combining remote work with in-personfield responsibilities.
Remote Work:
The Case Manager primarily works from home whennot conducting patient visits or attending required in-personmeetings with the team or supervisor.
Travel Requirements:
Regular in-person home visits arerequired, averaging 35-40 visits per quarter within an average30-mile radius of Waynesville, NC. The majority of visits takeplace in private residences, with occasional trips to medicalfacilities.
Scheduling & Autonomy:
Case Managers have flexibility insetting their travel schedules, coordinating directly with patientswhile ensuring all required visits and documentation arecompleted.
Meetings & Collaboration:
Employees must attend periodicin-person team meetings and supervisory meetings as scheduled.
Personal Vehicle Use:
Case Managers must use their personalvehicle for travel, with mileage reimbursement provided forwork-related trips.
Work Environment Considerations:
Travel includes rurallocations, which may involve unpredictable home environments andvarying road conditions.
Technology Requirements:
Since much of the work is conductedremotely, reliable internet access, proficiency in electronicdocumentation, and the ability to communicate effectively via phoneand virtual platforms are essential.
