Our Brands Responsibilities:
The Foster Care Case Manager is responsible foroverseeing the service planning, counseling, supervision, and casemanagement of children and families within the foster care system.
This role is critical in developing individualized treatment plans,coordinating comprehensive services, and ensuring that fosterchildren receive the medical, social, educational, and otherservices they need. Develop and implement individualized treatment plans forchildren in foster care. Prepare and maintain detailed case records and complete weeklyactivities/status reports. Coordinate and link children to appropriate services tominimize care fragmentation and ensure continuous access tonecessary services. Facilitate relationships and interactions between birthparents, foster families, and children to support family successand child welfare. Monitor and continually evaluate each child’s care plan,adjusting strategies as necessary to meet evolving needs. Assist in the recruitment, assessment, and training of fosterparents. Engage in collaborative relationships with foster parents tohelp them provide a safe, nurturing environment for fosterchildren. Coordinate services and service planning with other agenciesinvolved with the child by arranging medical, remedial, and dentalservices as needed. Collaborate closely with family assessment and planning teamsto prepare all case plans and coordinate services. Ensure all documentation is completed and submitted withinestablished timeframes in accordance with company policy. Maintain compliance with Health Connect America (HCA), State,and Council on Accreditation (COA) health and safetyrequirements. Resolve stakeholder issues promptly and proactively to avoidcomplications. Maintain high-quality work standards, focusing on continuousimprovement and quality. Communicate effectively with all stakeholders, keeping theminformed and updated. Collaborate effectively within the team, accepting feedbackpositively and contributing to team efficiency.
Qualifications:
Bachelor’s degree in social work or a related field such associology, psychology, special education, or counseling. At least two (2) years of experience in providing caseworkservices to children and families. Be Well with
