Salary listed is base on an entry•level candidate. Salary offer may increase according to case management experience. Must have earned a degree from an accredited college in a human service field. Two years work experience in human services field is recommended but may be waived or altered by the superintendent. Must maintain a work relationship with all students and staff. Responsibilities Client Service Responsibilities The Case Manager is responsible for preparing and maintaining permanent files on each child at Falcon Children’s Home. The Case Manager is responsible for preparing progress reports and plans of care and reviewing Social service needs on each child at Falcon Children’s Home. The Case Manager is responsible for assisting in defining social Service needs, formulation service plans and contract negotiation with each child regarding these goals and needs. This will be done through staffing with houseparents, social agencies and families to help them understand and define the needs and progress of the child. This will be done through counseling with each child to provide a cooperative effort at meeting his or her needs. The Case Manager is responsible for assisting in the counseling of the students concerning problem areas at Falcon Children’s Home, school and family. The Case Manager will be responsible for assisting the Director of Social Service with general office duties such as typing, filing and correspondence. Agency referral Responsibility The Case Manager will be responsible for suggesting agency referrals when appropriate to the plan of care for the child. When the agency involved is Mental Health, Developmental Evaluation or an auxiliary treatment center, the referral will be made by the Director of Social Services for final approval after consultation and/or recommendations in leading to the actual referral. When the agency involved is for medical or dental treatment or evaluation, referral will be coordinated by the Case Manager through the nurse, Director of Social Services or Administrative Assistant. The Case Manager is responsible for helping to maintain contact with the North Carolina Department of Human Resources regarding cases received through interstate compact. The Case Manager is responsible for any duty he/she is appointed to do by the administration. The Case Manager is directly amenable to the Social Service Director.
