POSITION OVERVIEW
The Housing Case Manager is responsible for providing one-on-one case management for individuals and families in need of crisis financial assistance for housing, utilities, and other emergency support. This role involves conducting homelessness prevention and rapid rehousing activities, assisting in Fayetteville Urban Ministry’s (FUM) food pantry and clothing closet, managing client files, and preparing various reports for internal and external stakeholders. The Housing Case Manager will work closely with the Emergency Assistance Coordinator, Emergency Assistance Aide, Chief Operations Officer (COO), volunteers, and community partners to ensure seamless service delivery and compliance with funding requirements.
KEY RESPONSIBILITIES
Case Management:
- Conduct intake assessments for clients seeking crisis financial assistance, ensuring timely and appropriate service delivery.
- Provide case management services to clients experiencing housing instability, including homelessness prevention, rapid rehousing, and assistance with utilities and emergency aid.
- Maintain and manage a client waitlist, reconciling files for accuracy and compliance with funding parameters.
- Conduct housing search and placement for eligible clients, ensuring suitable housing solutions are found.
Reporting:
- Prepare and submit quarterly reports for funding grantors, providing data on program performance, financial expenditures, and client outcomes.
- Create monthly reports, including financial data and client success stories, for the COO to present to the Board of Directors.
Community and Stakeholder Engagement:
- Attend community meetings with local partners, including the Fayetteville/Cumberland County Continuum of Care on Homelessness, and other stakeholders.
- Represent FUM at collaborative meetings and engage with stakeholders to identify opportunities for partnership and resource sharing.
Program Support and Administration:
- Assist with operations of FUM’s food pantry and clothing closet, ensuring these services complement case management activities.
- Manage client files to ensure compliance with organizational standards and funding guidelines.
- Participate in organizational leadership meetings and assist with annual fundraising events.
Team Collaboration and Leadership:
- Work closely with the COO to ensure that organizational objectives are met efficiently and effectively.
- Supervise and support volunteers and student interns who assist in Emergency Assistance programs.
Other Duties:
- Assist with organizational fundraising events and contribute to the success of FUM’s annual initiatives.
- Attend training and professional development opportunities as required.
Qualifications:
Bachelor’s degree in Social Work, Human Services, or a related field preferred. Minimum of 2-3 years of experience in case management, housing services, or related areas. Strong organizational and communication skills, with attention to detail in report preparation. Ability to work independently and as part of a team, with a passion for serving individuals and families in need. Ability to maintain professionalism and protect client privacy. High emotional intelligence a must. Proficiency in Microsoft Office Knowledge of community resources and housing programs is a plus.
Working Conditions:
Full-time position with occasional evening or weekend hours for community meetings or special events. Primarily office-based, with some local travel for meetings and client home visits.
