Ability to accurately and thoroughly complete the screening assessment during the interview; prioritize those with the highest acuity to refer for limited housing resources and manage the Continuum of Care Case Conferencing.
Essential Job Responsibilities:
Responsible for completing intake assessment with 95% accuracy. Conduct outreach to locations where unhoused persons can be located. Network with community stakeholders and providers. Determine homelessness based on the definition by the Department of Housing and Urban Development. Interview applicants with use of screening assessment to prioritize need. Refer applicant by warm-handoff referral to appropriate agency. Add clients to Case Conferencing. Provide Care Coordination through case management. Evaluate and make recommendations to administration based on trends and analyses of reports. Participate in CHN activities, staff and department meetings. Abide by all CHN, federal, state and county policies, procedures and mandates. Perform and other related duties as assigned. Always represent the agency in a positive manner.
Essential Job Qualifications:
Bachelor’s degree or equivalent reading skills required. 1-year experience in a customer service environment. Ability to communicate effectively with the public and applicants. Ability to maintain a positive demeanor. Flexible under pressure. Strong communication and communication skills. Experience working with the unhoused and low-income preferred.
