Full Job Description Summary:
Monitors services provided to Bethesda Center For the Homeless (BCH) guests, specifically for guests transitioning to permanent housing. Caseload can include assisting up to 25 guests.
Essential Functions:
Provides case management services including: assessments, service planning, monitoring, follow-up, counseling in coping with and resolving crisis situations. Guides guests in decision making and strengthening their existing support network. Identifies and collaborates with various community supporters, including housing managers, community agencies, service providers, and natural support systems. Assists guests in determining eligibility for financial assistance, social services, disability benefits, and on disability. Conduct tours of the community agencies and complete necessary applications as needed. Provides employment assistance and services for those qualified to obtain and retain employment. Documents guest activity and maintains records/progress notes according to guidelines. Documents in Homeless Management Information System (HMIS). Monitors services provided to the guest to evaluate effectiveness in meeting service needs. Participates in program evaluation and planning with staff. Collaborates with Case Management team to provide learning opportunities for guests. Creates guest activity reports for federal requirements and other funding sources. Occasionally participates in special events, fundraising and public presentations. Maintains an active interest and involvement in issues related to the homeless, substance use and mental illness. Ensures that first-rate customer service is top priority for all guests and supporters of BCH. Encourages a harmonious work environment by use of effective communication skills with co-workers, community resources, and guests. Assures quality and consistent services by being knowledgeable of and enforcing agency policies. Maintains professional competency by participating in field related up-to-date continued training, education and personal growth experiences. Promotes a positive image of BCH by ensuring an understanding of program services available and conducting oneself according to a professional code of ethics. Performs such other duties as incumbent upon the position and as assigned.
Knowledge, Skills & Abilities:
Bachelor’s degree in social work, psychology, or an Associate’s degree with relevant years of work history or related human service field. Minimum of three years’ work experience with chronically homeless, substance abuse, mentally ill, or social work. Strong commitment to serving the needs of disadvantaged adults. Articulates ideas clearly (both written and orally), listens accurately, and respects/understands social, cultural and racial differences. Computer proficiency and technical aptitude with knowledge of MS Office applications. Maintains confidentiality and HIPAA compliance at all times. Confident personality and the ability to build rapport among the guests and the service providers. Ability to effectively multi-task in a fast paced environment. Maintains dignity and self-control in stressful situations. Professional, pleasant attitude and dress. Knowledge of community resources and social service agencies within the community. Understanding of Housing and Urban Development (HUD) guidelines and evidence based models for best practices, Critical Time Intervention and Housing First.
Compensation:
$45,000-50,000
Expected Hours of Work:
Regular days and hours of work are Monday through Friday, 8:00 am to 5:00pm. Works flexible hours for meetings, events, or other agency functions when needed.
Work Environment and Physical Requirements:
Maintains physical condition appropriate to the performance of assigned duties and responsibilities, which may include the following: walking, bending, stretching, lifting, standing or sitting for extended periods of time, operating assigned equipment. Work conditions sometimes include assisting guests with moving. This may involve lifting boxes and furniture, and occasionally working outdoors. Exposure to unpredictable and potentially violent guests. Ability to work flexible/overtime hours, as needed. Requires valid NC driver’s license and ability to travel between various agency sites, follow-up meetings, and community resources, as well as transporting guests.
