Position Overview The Case Manager (CM) is a vital role in managing resident services and programs for seamless operation. This position provides comprehensive case management for mothers in recovery and their children. This support allows them to improve their quality of life and maximize their ability to function within the community. The CM collaborates with community service agencies to ensure residents receive necessary case management services. The ideal candidate is someone with deep interest and commitment to helping those affected by substance use disorder, both directly and indirectly. The candidate must possess critical thinking skills and the ability to respond quickly and adapt to unexpected circumstances. Key Responsibilities Review applications, schedule and conduct comprehensive screenings, and manage the intake process for new residents, ensuring accurate documentation and a smooth transition into the recovery program. Coordinate and schedule essential resident services, including transportation to appointments, court appearances, and program activities. Pick up, distribute, and document resident medications, adhering to established protocols and ensuring compliance with healthcare provider instructions. Provide ongoing support to residents in adhering to their individualized recovery plans. Maintain accurate and up-to-date resident records, including progress notes, incident reports, and medication logs, while adhering to confidentiality guidelines and reporting requirements Qualifications & Experience Bachelor’s degree or equivalent work experience Experience working with at-risk families Excellent oral and written communication skills Knowledge of addiction and treatment methods used Ability to assess and prioritize the needs of residents Self-motivated, dependable Ability to use standard office equipment including fax, computer, and copier Valid North Carolina driver’s license and good driving record Benefits Competitive salary based on experience Paid time off and holidays Employee discounts IRA Match Cell phone allowance
