JOB GENERAL STATEMENT OF DUTIES
This employee determines, documents, and ensures applicants meet all program integrity requirements necessary for eligibility every six months to one year for WIC program participants. The employee verifies and documents proof of identity, residency, and income for applicants based on acceptable identification as approved by WIC regulations. Verifies and documents proof of income for every member of the applicant’s household, calculates gross income per household unit, and uses professional judgment to determine family size also based on WIC regulations. Supervision is received through individual conferences and unit meetings and by review and evaluation of cases.

DUTIES AND RESPONSIBILITIES
Essential Duties and TasksInitiates patient master record at the initial appointment for WIC services. Interviews patient and enters demographic information into the system.
Documents required information regarding voucher issuance into the system and issuance logs.
Obtains participant signatures to document receipt of vouchers, or when vouchers are lost, stolen, voided, and replaced. Schedules subsequent appointments for required assessment and counseling. Follow-up appointments within required time frame for individual assessment and counseling or nutrition classes.
Instructs participant classes on proper redemption of vouchers at vendor establishments.
Teaches mini nutrition lessons to individual participants at voucher issuance.
Mails vouchers to participants with serious medical conditions, faxes information to facilitate transfers, and copies required documentation.
Protects confidential patient information.
Communicates and offers input regarding clinic protocol and procedures.
Completes annual individual performance agreements to enhance program services.
Assists with outreach efforts to increase program awareness, by contacting other agencies and mailing outreach packets.
Verifies and completes program integrity for MCC/WIC certifications and mails food instruments to clients.
Contact MCC, CSC and other staff to facilitate delivery of other health department services.

Ability to compose and gather a variety of materials independently.
Ability to plan and organize work independently.
Ability to document complex situations in a comprehensive and legible manner.
Ability to add, subtract, multiply, and divide whole numbers and fractions, including use of decimals and computation of percentages.
Ability to plan and organize work and time in an efficient manner which will result in applications, reviews, desk revisions, etc. being processed by program deadlines.
SUPPLEMENTAL INFORMATION
Physical Demands:
Must be able to physically perform the basic life operational functions of fingering, grasping, talking, hearing, and repetitive motion. Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. Must possess the visual acuity to prepare and analyze data, operate a computer terminal, and do extensive reading.
May Require Driving:
This position may require driving for this position whether driving a County owned or personal vehicle to conduct county business such as but not limited to attending conferences, meetings, or any other county related functions. Motor Vehicle Reports may be verified for valid driver’s license and that the driving record is compatible with the county’s driving criteria. If a personal vehicle is operated for county business proper insurance is maintained as per Guilford County’s vehicle use policy.
